By completing this form, you acknowledge and understand that the original license/term will be canceled, and the updated license/term will start on the effective date stated below for the full term.

❖ For Upgrades, Term Updates, Consolidation of Organizations, and Subscription Model Change, a prorated credit will be calculated based on the amount paid on the original license items and the remaining term, and the credit will be applied to the new subscription invoice.
